TITLE: MRI Center Staff Nurse

CLASSIFICATION : Registered Nurse (RN)

GENERAL POSITION SUMMARY: To render highly professional and technical nursing care to assigned MRI patients and their family. Provide direct or indirect total patient care requiring evaluative judgment, specialized knowledge and professional nursing skills. Collaborates with radiologists, technologists and other staff to insure the highest standards of care in a safe and efficient manner.

MAJOR AREAS OF RESPONSIBILITY:

  • Sedation and medication administration; injection of IV contrast; patient monitoring and recovery
  • MRI patient relat! ions and throughput
  • Patient safety
  • Staff and! patient/family education
  • Policies and procedures (facility and nursing)
  • Maintaining facility and staff readiness for daily and emergent interventions.
  • Assist radiologist in emergency situations.
DUTIES AND RESPONSIBILITIES:
  • Completes the nursing process by evaluating the physiologic and psychologic needs of the patient before, during and after the MRI scan. Obtains thorough and accurate medical histories from the patients who may need medication for sedation to undergo MRI. Identifies actual and potential problems, interventions and desired outcomes.
  • Evaluates the patient's progress and collaborates with the technologists and radiologists to insure diagnostic imaging.
  • Monitors and documents vital signs and assessment per sedation protocol on all medicated patients.
  • Monitoring/Observation of non-sedated patients requiring additional assistance (inpatients are monitored by floor RN).
  • Discusses the pending sedation patient's medical history, pre-sedation vital signs and safety of potential sedation with the site appointed radiologist. Obtains the physician’s orders and has full knowledge of drugs, routes and side effects, and administers medications as prescribed.
  • Documents all orders. Monitors patients throughout and communicates all abnormal patient responses to M.D. promptly.
  • Explains procedure and gives medications ordered to the patients to gain cooperation and allay their concerns and apprehension.
  • Performs venipuncture and inserts intravenous lines for sedation and contrast injections or butterfly-type needle for contrast injections. Follows proper nursing techniques and OSHA safety guidelines when performing venipuncture work.
  • Perform I-STAT Creatinine draws/tests as needed and document results in RIS Patient/Visit Notes.
  • Observes patients for reactions to medication and treatment and follows up wi! th appropriate nursing care.
  • Co-writes, reviews and evaluates ! specific MRI policies and procedures. This includes routines for: scheduling patients requiring sedation; medication administration; assisting the claustrophobic patients; effective scanning of the pediatric patient; assistance of general anesthesia; patient safety and emergency interventions.
  • For patients requiring assistance due to claustrophobia, develop a trusting relationship with the patients and attempt to enable effective scanning by staying with the patient throughout the scan. Under the direction of the technologist, allow patient to move out of the scanner in-between scans for rest and reassurance. Should these measures fail, carry out sedation orders.
  • Have knowledge of intravenous contrast media used in MRI and effectively administer it as needed. Educates patient prior to this procedure. Troubleshoots and assures proper IV access and side effects. Treats per contrast reaction protocol and documents any known or possible contrast reaction and s! ubmits to RN Supervisor for FDA reporting.
  • Serves as patient advocate at all times. Greets patients, refers to them by name in a courteous manner, reviews patient history form or helps complete and troubleshoots potential problems. Assists patient to dressing area and instructs on changing attire. Discusses scan and what to expect and addresses any questions or concerns that patient/family may have. Always maintains patient dignity, confidentiality and safety.
  • Assists in the goals of the MRI Center. Attends a minimum of 3 out of 4 scheduled quarterly staff meetings, participates in meetings, etc. Acts as member of a team by communicating effectively and assisting others whenever possible.
  • Stays current with daily schedule and works with the scheduling coordinators to fill any openings. Assists the scheduling coordinators with pre-screening unusual requests or assessment of status to undergo MRI scanning based on mental and physical conditions, pre! sence of IV's, oxygen, monitors, weight and size of patient or any othe! r special needs. Confers with staff radiologists, technologists, manager or patient's physician when needed.
  • Keeps up to date on types of supplies and medications needed for daily and emergency measures. Orders, stocks and maintains all drugs, including scheduled drugs normally required. Maintains proper drug log and records according to DEA regulations. Assist with medication ordering, including DEA 222 forms.
  • In the event a patient requires anesthesiology to assist with an MRI case, makes all necessary arrangements with the anesthesiologist, surgery scheduling, referring physician, radiologist and all other health professionals involved. Assists the anesthetist during the scan when needed, works with family and recovery room staff for post scan follow through. Obtains H & P from ordering physician and makes sure anesthesia has it readily available on patient chart.
  • In the event that a child requiring sedation is scheduled, call the parents at le! ast 24 hours prior to the scheduled scan and introduce self, routine and determine need for sedation via parents interview. Discuss instructions, possible side effects and provide directions and pre-scan screening.
  • In the event that a known claustrophobic patient is scheduled or a patient known to be uncooperative or unable to lie flat for their scan is scheduled, call the patient and pre- interview and instruct as noted in the above paragraph.
  • Follow proper OSHA guidelines, insuring proper handling and disposal of contaminated waste.
  • Assists in services when needed if within RN scope of practice, including but not limited to patient positioning and table preparation.
  • Discuss care and preparation for inpatients with RN on floor. Give rationale for NPO status, necessary removal of IV pumps and monitoring equipment. Provide follow-up to patient floor regarding medications administered and/or other pertinent patient care information.
  • Follows all compliance policies, procedures and guidelines, including! promptly reporting any compliance violation.
  • Other duties as required.
EDUCATION, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS :
  • School of nursing graduate with 2 years nursing experience. Back ground experience should include minimum of one year critical care experience, working with pediatric patients, sedation administration and monitoring.
  • Current licensure in the State of Indiana.
  • CPR certified and ACLS certified/eligible to be certified within six months. PALS certified/eligible.
  • Basic knowledge of EKG interpretation.
  • Familiarity with the following equipment: EKG monitor, defibrillator, all emergency equipment, O2, suction equipment, blood pressure equipment, pulse oximeter (SAO2 monitor), angiocatheter needles.
  • Familiarity with all drugs used for sedation, indications, dosages, side effects and treatment of side effects.
  • Familiarity with narcotic and benzodiazepine revers! al agents and side effects.
  • Good communication and interpersonal skills in interactions with patients, their families, staff, physicians and others.
  • Recognizes errors promptly and responds in accordance with policy or supervisor to rectify. Demonstrates accountability for errors made.
  • IV therapy skills - pediatric and adult to include use of PICC lines, temporary central venous catheters and implanted central lines.
  • TB certified/eligible within six months.
WORKING CONDITIONS: Primarily works in normal hospital/clinic environment with some work time required in the area of a strong magnetic field. Able to tolerate long periods of walking, sitting and standing. Driving between five MRI locations will be required at times. Moderately stressful at times due to multiple demands and ability to respond immediately in an emergency situation. Potential hazard due to direct patient contact and potential exposure to patients with i! nfections, diseases, hostile or incoherent patients and electrical equi! pment.

AUTHORITY BOUNDARIES: Handles all problems and questions within limits of knowledge and experience. Follows orders or physician approved guidelines for drug administration. Makes recommendation to supervisor regarding policy and procedures changes/improvements. Reports all issues outside scope of responsibility.
